Onboarding Form

After selecting your state and speaking with a SimplyTest representative, you’ll complete a New Client Onboarding Form.

For your onboarding form, you will need.

  • Practice details (name, address, contact)
  • Provider NPI / tax ID
  • Accounts Payable Information
  • Billing & Shipping Address
